Our evaluation report on Sunderland All Together Better has recently been published. This is one of the NHS's Multi-speciality Community Provider (MCP) Vanguard Programmes. The model aims to bring together health and social care teams, in addition to the voluntary and community sector, in order to achieve better outcomes for service users and patients, professional staff, and the overall health and social care system in Sunderland.
