The Gwent Frailty Partnership commissioned Cordis Bright to undertake a review of the Gwent Frailty Programme, a partnership between Aneurin Bevan University Health Board and Blaenau Gwent, Caerphilly, Monmouthshire, Newport and Torfaen local authorities. This report sets out the findings from the review, which took place between March and June 2014, and makes a number of recommendations for taking forward the project.
